All official Scouting activities require an up-to-date Annual Health and Medical Record (aka "Medical Form").  Download a pdf version at the links below:


Parts A & B - No physician signature required (for activities of less than 72 hours duration )


Parts A, B, & C - Physician signature required on Part C (for activities of 72 hours duration or longer)


If you are attending a high adventure base, use these:  Florida Sea Base (scuba divers: get the PADI statement, too),  Northern TierPhilmont Scout Ranch (PTC Backcountry Attendees: get this one, too),  or Summit Bechtel Reserve
